Camp Registration Camper Release Form
This link should be shared with parents to complete. Please read important directions below!
1. Please make sure parents read the directions and complete the "CAMPER" forms for anyone under 18. If they select "Sponsor" and complete those forms it will invalidate the forms and I will have to delete it and they will need to completely redo the form.
2. Parents MUST save the link confirming they competed the forms. If anything changes such as medication, insurance info, medical info etc, they can click that link and edit the form themselves. Please make sure they save this!
3. If campers will be leaving at any point during camp, plese make sure parents complete the form attached and bring it with you to camp check-in.

4. ALL sponsors over the age of 18 must have a completed Child Protection
Training certificate printed and turned in at camp check-in. Certificates

5. ALL your sponsors must be on the sponsor verification form that is
signed by your pastor, and your church must have current background checks
on file for anyone over 18. You don't need to produce the background
checks to the camp, just make sure they are on file at your church.
6. Church leaders will be receiving a separate email with a spreadsheet
just for your church showing who is registered and their basic info.
